Today, being [[date_today]], I authorize Pivotal Home Solutions, located at 1751 West Diehl Road , Naperville, Illinois, to charge the credit card or electronically debit the bank account, as selected by me, to pay the current balance on my Pivotal Home Solutions accounts and to enroll me on the Pivotal Home Solutions autopay program, by which Pivotal Home Solutions will charge such credit card or electronically debit such bank account, in the fixed amount agreed upon during my Pivotal Home Solutions enrollment and either monthly or annually, as agreed upon during my [[brand_name]] enrollment, unless [[brand_name]] notifies me of a variation in this payment amount.
I understand that I have a right to receive updated card account information and revocation/cancellation policies upon request to Pivotal Home Solutions. I understand that the autopay program charge will be made on the date that is three days prior to the due date on my bill, and payments will be processed within 1 to 3 banking days. I also authorize Pivotal Home Solutions or its service provider to electronically credit or debit my bank account to correct erroneous debits and to update my payment card information on file when it is set to expire or based on updates received from the bank that issues my payment card. If an attempted debit to the identified bank account is returned unpaid, I authorize Pivotal Home Solutions to collect any applicable late fees or return items fees by electronic funds transfer.
I understand that my authorization starts today and will remain in full force and effect until I revoke this authorization by phone at 855-890-8792, in writing at 1751 W. Diehl Rd., Ste 200, Naperville, IL 60563, or online at www.yourhomesolutions.com. I understand that [[brand_name]] must receive my request at least three (3) business days before the next scheduled date of a transfer. I understand that [[brand_name]] and/or my financial institution can cancel my enrollment in this program at any time.
